Job description
- Enables consumers to stay in their homes.
- Follows care plan as directed by designated supervisor.
- Monitors consumers condition by observing physical and mental condition, intake and output, and exercise.
- Supports consumers by providing light housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands.
- Assists consumers by providing personal care services, such as bathing, dressing, and grooming.
- Helps consumers care for themselves by assisting with the use of cane or walker, special utensils to eat, adaptive equipment, reminders with medication and person-centered support for personal hygiene.
- Notifies designated supervisor of changes, concerns, or unusual conditions with consumer.
- Maintains a safe, secure, and healthy environment.
- Maintains security precautions.
- Follows prescribed dietary requirements and nutrition standards.
Job qualifications
Completion of PHCAST PC/HM course, A valid drivers' license, ability to read, understand, and follow a clients' plan of care, must be at least 18 years of age, Experience providing care to others, High School diploma or equivalency preferred. Must be able to communicate with office via phone.
