Essential Duties and Responsibilities include the following: Other duties may be assigned:
- Enables consumers to stay in their homes.
- Assist the client in the Activities of Daily Living (ADL) including personal care, hygiene, baths, shampoo, skin and nail care and Ambulation.
- Assist Client with ambulation and transferring. Client must be 50% weight with ambulation and transfers.
- Transfer Examples:
- Bed to Chair/Wheelchair and vice versa, in and out of shower/tub, commode
- Transfer Examples:
- Follows care plan as directed by designated supervisor.
- Monitors consumers condition by observing physical and mental condition, intake and output, and exercise.
- Supports consumers by providing light housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands.
- Assists consumers by providing personal care services, such as bathing, dressing, and grooming.
- Helps consumers care for themselves by assisting with the use of cane or walker, special utensils to eat, adaptive equipment, reminders with medication and person-centered support for personal hygiene.
- Notifies designated supervisor of changes, concerns, or unusual conditions with consumer.
- Maintains a safe, secure, and healthy environment.
Maintains security precautions.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Must be Dependable, Compassionate, Responsible, Flexible, have a Great Work Ethics and a Friendly Personality.
Must be 18 Years old or older
Must have a Valid Driver's License (In State of Residency)

All Canidates must be able to pass a Cori, References and Multi-State backgrounds.
Proof of up-dated (TB) Tuberculosis (Negative Results) Copy of X-Ray results if a (Positive Result)
Physical Examination State Form: (Provided my NBHC office) Stating Employee is in Good Health and physically able to perform the responsibilities of the above named job
