The personal care assistant / homemaker is a paraprofessional caregiver who works in the home under the periodic supervision of a licensed nurse to provide personal care services, and performs assigned light housekeeping duties.

• Enables consumers to stay in their homes.
• Follows care plan as directed by designated supervisor.
• Monitors consumers condition by observing physical and mental condition, intake and output, and exercise.
• Supports consumers by providing light housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands.
• Assists consumers by providing personal care services, such as bathing, dressing, and grooming.
• Helps consumers care for themselves by assisting with the use of cane or walker, special utensils to eat, adaptive equipment, reminders with medication and person-centered support for personal hygiene.
• Notifies designated supervisor of changes, concerns, or unusual conditions with consumer.
• Maintains a safe, secure, and healthy environment.
• Maintains security precautions.
• Follows prescribed dietary requirements and nutrition standards.
